Sexual Misconduct Reporting Form

If you, or someone you know is imminent danger to themselves or danger to someone else, immediately call 911

Individuals are encouraged to report all forms of sex-based misconduct, no matter the severity so they can receive support and the university can respond appropriately. NWOSU’s primary concern is safety; therefore, individuals should not be deterred from reporting for any reason, even if the use of alcohol or other drugs were involved.

You may report an incident of sexual misconduct anonymously by excluding the “Reporting Party Information” section. Anonymous reporting may inherently limit the scope of the investigation.
NWOSU strongly encourages individuals to report any instance of sexual violence to the police.

The criminal investigation, should the complainant choose to pursue criminal prosecution, will be conducted by law enforcement. This form does not constitute filing a criminal complaint with the police. To file an official police report, please contact Campus Police or your local law enforcement agency. If this is an emergency, immediately call 911

Once the form is submitted, it will be sent to the Title IX office for review. Submissions of this form may not be reviewed outside of normal business hours.
