Art Show Entry Form

Once you have filled out the form below, you will need to prepare your artwork paying close attention to how you are presenting it. Mounting, matting, framing, etc., are an important part of your overall art piece. Do not put your photo entries in a frame - only mount or mat.

The deadline to submit this form is Feb. 20. Please bring your artwork to the J.W. Martin Library on the Alva campus on Feb. 27 from 8 a.m. to 5 p.m. You will take it to the Archives Room, upstairs in the library. Please ask for directions. The show will be March 3-29.

The NWOSU Art Society and the J.W. Martin Library are not responsible for loss or damage of any artwork. Any work deemed inappropriate by NAS and school standards will not be displayed.

Artist Information

Entry Information -- Please number each line and provide the name of the Category (Artisan Crafts, Fiber Art, Quilting, Cartoon/Comics, Digital Art, Mixed Media/Collage), the name of your art piece and what size it is. (Example: 1. Mixed Media/Collage, "Fall Scene," 8x10, 2. Digital Art, "Barn," 16x20)

Entry Information -- Please number each line and provide the name of the Category in Painting (Watercolor, Animals/Creatures, Architecture, Elements of Design, The Figure/Portraiture, Landscapes/Nature, All other subject areas other than those listed), the name of your art piece and what size it is. (Example: 1. Watercolor, "Snowman," 11x14, 2. Architecture, "Skyscraper," 16x20)

Entry Information -- Please number each line and provide the name of the Category in Works on Paper (Watercolor, Animals/Creatures, Architecture, Elements of Design, The Figure/Portraiture, Landscapes/Nature, All other subject areas other than those listed), the name of your art piece and what size it is. (Example: 1. Elements of Design, "Lines and Shapes," 16x20, 2. Landscapes/Nature, "Rolling Hills," 16x20)

Entry Information -- Please number each line and provide the name of the Category in Photography (Animals/Creature, Black and White, Landscapes, Cityscapes, Light, Elements of Design, Macro, People, Nature, All other subject areas other than those already listed), the name of your art piece and what size it is. (Example: 1. Macro, "Texture," 8x10, 2. People, "Henry," 8x10)

By choosing "YES," you agree that you have read and understand all rules, information, requirements, category descriptions, etc., for entering the NAS Art Show, and agree to terms that NWOSU, NAS and SGA are not held responsible for any damages or loss to artwork. If you choose "NO," your artwork will not be displayed.

Permission Signature *

By choosing "YES," if your artwork receives any prize, you will allow your artwork to be moved to the Winners' Show in downtown Alva. By choosing "NO," your winning artwork will not be taken downtown, and it can be picked up at the close of the show in the library.
