Nomination Form

Each year the University asks for nominations to honor an outstanding teacher through the John Barton Distinguished Teaching and Service Award.  Below is a form you may use to enter whom you think is deserving of such an award.

Please return this form via campus mail to the NWOSU Foundation office.

 THE JOHN BARTON DISTINGUISHED TEACHING AND SERVICE AWARD

Nominations will be accepted until 5 p.m. Monday, October 9, for the annual John Barton Distinguished Teaching and Service Award.  The nominee must be a full time employee of Northwestern, with at least a one-half time teaching load.  The nominee must also exemplify distinguished teaching and administrative leadership.

The award carries an honorarium and a plaque, which will be presented to the winner at the NWOSU Faculty and Staff Recognition in November.  Only one nomination may be submitted by an individual.
